What are All-on-4 dental implants?

All-on-4 dental implants provide a fixed and long-term solution for missing teeth and are a superior alternative to full dentures. This treatment is designed for people who are about to lose or have already lost all of their teeth in their upper or lower jaw (or both). It involves the placement of four titanium implants in the jaw that secure a permanently fixed tooth bridge of up to 12 teeth in place.

The beauty of All-on-4 dental implants is that the tooth bridge can be attached two to three days after the implant surgery. Unlike other tooth replacement treatments, you do not have to wait months before your new teeth are fitted. With All-on-4 dental implants, you can restore the appearance and function of your smile in just days.

How do All-on-4 dental implants work?

The All-on-4 dental implants treatment concept was created by Nobel Biocare and is clinically proven with a 95-97% success rate. The treatment uses a special technique for the placement of four titanium implants, with two straight implants placed at the front of the jaw and two angled  implants placed at the back of the jaw. The implants at the back of the jaw are placed at approximately 30 - 45 degree angle. This technique helps to overcome bone deficiencies and utilises more bone density to help support the tooth bridge. In most cases, it allows a tooth bridge of at least 12 teeth to be fitted without the need for bone grafting, even if a patient has jaw bone loss. Over time, the jaw bone fuses with the All-on-4 dental implants, adding further strength and support for the tooth bridge.

How much does All-on-4 cost?

The cost of All on 4 treatment will vary, depending on your specific needs and the type of tooth bridge you select for your new teeth. To work out an estimated cost for your All on 4 treatment at Macquarie Street Dental, start with the baseline cost (from $18,000 p/jaw for implant placement and temporary tooth bridge) then add the cost of your preferred final tooth bridge. What’s involved in the two stages of our All-on-4 treatment?

Our All-on-4 treatment is delivered in two stages. Stage 1 includes the implant placement and the temporary tooth bridge, which will be worn during your healing period (approx. 6 months). In Stage 2 (once you’ve healed), you’ll be fitted with your final tooth bridge — a more durable, definitive solution, designed to have the most natural-looking finish and stand the test of time.

Why does Dr Dunn only offer a two-bridge solution for All-on-4 treatment?

At Macquarie Street Dental, we only offer and recommend a two-bridge solution because it has shown to be the most effective approach to All-on-4 treatment. During the healing period, the structure of your jaw and gums will change. This remodelling of your jaw bone is a perfectly natural and expected response to the implant surgery. However, this means that the temporary tooth bridge placed after your surgery may no longer fit as snugly or comfortably once you’re healed. An ill-fitting tooth bridge may hinder your ability to speak, eat and maintain your oral health effectively, putting you at risk of All-on-4 complications. It can also leave gaps between your gums and tooth bridge where food can get stuck. This is why we only place your final tooth bridge once your jaw and gums are completely healed, ensuring your new teeth are the perfect fit for your mouth.

How long does it take to get All-on-4 dental implants?

At your initial consultation, Dr Dunn will advise on your exact treatment timeline and the date of your implant surgery. On the day of your dental implant surgery, the procedure will take approximately 3-4 hours per jaw.

In the following days, you’ll return to our dental implants centre to have your new temporary tooth bridge tried in and then  fixed to your All-on-4 dental implants. You will receive the Starter acrylic tooth bridge first and wear this for around 6 months while you heal. You will then upgrade to our Classic, Signature or Statement tooth bridge.

How painful are All-on-4 dental implants?

Ideally, your All-on-4 dental implant surgery will be performed under sedation and anaesthetic will be used to numb your mouth. This means you should not feel any pain or discomfort during the procedure. After surgery, you may experience some discomfort but this can be easily managed with oral pain relief medication. Fitting your new tooth bridge to your dental implants is usually painless, but if required, a local anaesthetic may be applied for added comfort.

Are All-on-4 dental implant tooth bridges removable?

Yes,  All-on-4 dental implants tooth bridge can be removed — but only by your dentist. Your All-on-4 tooth bridge may need to be removed every few years for regular cleaning and maintenance.

Does food get stuck under All-on-4 dental implants?

If your All-on-4 dental implants treatment is done properly, your tooth bridge should rest snugly on top of your gums, leaving very minimal space for food to get stuck. However, it’s important that you maintain good oral hygiene and take care of your All-on-4 dental implant  tooth bridges as you would natural teeth.

You’ll need to ensure proper daily cleaning and maintenance to help clear away any food particles and plaque build up and help ensure your All-on-4 dental implants lasts as long as possible. This includes:

  • Brushing your teeth and gums
  • Flossing under your tooth bridge
  • Using a water flosser — Dr Dunn provides all of his patients with a Waterpik to help with cleaning.

How long do All-on-4 dental implants last?

When cared for correctly, your All-on-4 dental implants should last several decades. The longevity of your tooth bridge will depend on the type of bridge you choose as well as your ongoing care and maintenance. Our Signature or Statement tooth bridges are our most durable options and typically last two decades or more with the right care. The Classic tooth bridge can last for up to 15 years, depending on proper care, as well as the impact of your bite and whether you have any grinding or bruxism issues. The Starter tooth bridge must be replaced after 12 months maximum.
